Use this enrolment form to provide the information we need to enrol you into a qualification at Ministry Training College. This form includes information required by the Ministry of Education and other Government agencies for statistical and registration reasons.

Please note - International Applicants: Applications from International Students MUST be sent to the Auckland Campus Principal only as our Christchurch campus will not be accepting applications from international students. A non-refundable fee of NZ$50.00 will apply for International Students.

Give this form to people who will reference your application to MTC.

Please note - All Applicants: Domestic applicants will need to get two people to complete the Applicant Reference Form and International applicants will need to get three people to complete the Applicant Reference Form. The Referees MUST send the forms directly to the Campus Principal.
